Job summary

One Senior Care is seeking an Enrollment Specialist to guide seniors and their families through the early enrollment steps. You will build relationships, complete assessments, coordinate documentation, and support participants through the enrollment process in homes and community settings.

  • Bachelor's degree preferred.
  • Experience with frail or elderly populations is a plus.
  • Knowledge of Medicare and Medicaid regulations is helpful.
  • Experience with community outreach, enrollment, sales, or marketing is a plus.

  • Meet with potential participants and families to explain the program and assess needs.
  • Complete in home and community based assessments for prospective participants.
  • Coordinate required evaluations, documentation, and intake steps.
  • Attend intake appointments and support participants throughout enrollment.
  • Complete intake and enrollment packets accurately and on time.
  • Serve as the main contact for referrals and guide them through enrollment stages.
  • Track enrollment progress and collaborate with the Interdisciplinary Team.

Job Summary

Duties and Responsibilities
  • Meet with potential participants and families to explain the program and assess needs
  • Complete in home and community based assessments for prospective Participants
  • Coordinate required evaluations documentation and intake steps
  • Attend intake appointments and support Participants throughout the enrollment process
  • Complete intake and enrollment packets accurately and on time
  • Serve as the main point of contact for referrals and guide them through each stage of enrollment
  • Present referrals to the Interdisciplinary Team for review and ongoing collaboration
  • Maintain accurate records and communication updates in the CRM and EMR
  • Work closely with the Medicaid Enrollment Specialist to support a smooth transition to Medicaid processing and verification
  • Participate in team meetings training and quality improvement activities
  • Track weekly and monthly goals enrollment progress and key performance metrics
  • Help identify barriers to enrollment and work with the team to create solutions
  • Support additional projects and responsibilities as assigned

Education and Experience
