About P.A.C.E.

The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, many provided onsite at a PACE Center. It is an alternative to nursing home care, coordinating preventive, primary, acute, and long-term care through an interdisciplinary care team (IDT) including physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides, enabling older individuals to live in the community.

Primary Purpose

Provide care services to participants, including assistance with shopping, errands, laundry, housekeeping, and meal prep within their homes, community settings, or day centers, following care plans.

Key Responsibilities
  • Assist with ADLs such as shopping, laundry, meal prep, medication reminders, etc., to support health and independence.
  • Observe, record, and report changes in participants' emotional, behavioral, and physical conditions.
  • Respectfully communicate to promote autonomy and dignity.
  • Encourage maximum independence during care.
  • Collaborate with staff and families using a participant-centered care model.
Qualifications
  • High school diploma or equivalent.
  • Discretion and confidentiality skills.
  • Good communication skills, both verbal and written.
  • Detail-oriented with problem-solving skills.
  • Basic computer skills.
  • At least one year of experience working with the elderly or in long-term care.
Work Environment
  • Communicates with staff, participants, caregivers, and organizations.
  • Willing to work beyond normal hours, including weekends and holidays.
  • Subject to emergency callbacks and potential exposure to infectious waste and diseases.
  • Must drive in various weather conditions.
  • Support diverse populations, including those with dementia or cognitive limitations.
  • Capable of handling mental and emotional stress.
  • Willing to work in varied home environments with potential hazards.
Physical Demands
  • Assist with daily activities, lifting up to 50 lbs., and other physical tasks.
  • Ability to work independently and effectively in various situations.
  • Assist in emergency evacuations if necessary.
Minimum Requirements
  • Reliable transportation, valid Michigan driver’s license, and insurance.
  • At least 18 years old.
  • Fully vaccinated or willing to be vaccinated against specified communicable diseases.
