CareMed Health Solutions seeks a part-time Wound Care Nurse Practitioner to manage wound care for residents in long-term care facilities. You will assess wounds, create treatment plans, and coordinate with the interdisciplinary team to optimize outcomes.

Responsibilities include prescribing medications, ordering/monitoring tests, and collaborating with the primary care physician to integrate the care plan. An OH NP license with board certification and wound care experience are required.

Formación

• Current, unrestricted NP license in OH.
• Board Certification.
• DEA licensure (Optional).
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care and related conditions.
• Comfort diagnosing and treating wounds from various etiologies.
• Familiar with wound care products and wound vacs.
• Experience in long-term care settings and EMR systems.

We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.

WOUND NP ROLE AND RESPONSIBILITIES:

• Provide wound care services for long-term care patients residing in skilled nursing facilities.
• Prescribe appropriate medications, including controlled substances in accordance with OH Law.
• Order, review, and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with the primary care physician.

WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of OH.
• Board Certification.
• DEA licensure (Optional)
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds
• Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Must be comfortable staging pressure injuries according to the NPIAP
• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
• Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices
• Strong preference to having wound certification or be open to getting certified
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to work within an interdisciplinary care team to coordinate patient care.
