Job summary

CareMed Health Solutions is seeking a Wound NP to provide specialized wound care services to residents in long-term care facilities in Ohio. You will collaborate with an interdisciplinary team to deliver comprehensive care and improve patient outcomes.

The ideal candidate holds an unrestricted Ohio NP license, board certification, and has demonstrated wound care experience in geriatric or adult populations. Prior use of EMR systems and familiarity with wound VACs are preferred.

Qualifications

• Must hold an active, unrestricted NP license in Ohio.
• Board certification required.
• DEA licensure optional per OH law.
• Experience with wound care in adults, geriatric or family practice.
• Comfort diagnosing and treating diabetic, arterial, venous and mixed wounds.
• Familiar with pressure injury staging per NPIAP.
• Knowledge of advanced wound care products and wound VACs.
• Experience in long-term care settings and EMR systems.

Responsibilities

• Provide wound care services to long-term care residents in skilled nursing facilities.
• Prescribe medications in accordance with Ohio law, including controlled substances.
• Order, review, and follow up on treatments and testing as needed.
• Communicate changes in patient conditions with nursing staff and care teams.
• Coordinate with the primary care physician and interdisciplinary team to optimize care.
• Participate in development and execution of care plans.

We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.

WOUND NP ROLE AND RESPONSIBILITIES

• Provide wound care services for long-term care patients residing in skilled nursing facilities.

• Prescribe appropriate medications, including controlled substances in accordance with OH Law.

• Order, review, and/or follow-up with treatments and diagnostic testing.

• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.

• and/or the care plan.

• Collaborate with the primary care physician.
  

WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of Ohio.

• Board Certification.

• DEA licensure (Optional)

• Clinical background in adult, family, or geriatric specialties.

• Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds

• Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.

• Must be comfortable staging pressure injuries according to the NPIAP

• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.

• Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices

• Strong preference to having wound certification or be open to getting certified

• Experience working in a long-term care setting.

• Ability to work with an EMR.

• Ability to work within an interdisciplinary care team to coordinate patient care
