Job summary

Avamere Transitional Care of Puget Sound in Tacoma, WA is seeking an LPN for a part-time Evening shift. You will provide essential nursing care under RN supervision, including vital signs, meds, wound care, and activities of daily living.

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Active LPN license in Washington state is required and current.
• CPR/BLS certification must be maintained during employment.
• Experience in long-term care, geriatric rehab, sub-acute or critical care is preferred.
• Experience with electronic medical records is preferred.

Responsibilities

• Provide basic nursing care including vital signs, medications, wound care, and daily activities under RN supervision.
• Measure, record, and monitor patient vital signs accurately.
• Administer medications per physician orders and facility protocol.
• Support hygiene, feeding, mobility, and daily living activities.
• Document care provided, observations, and any changes in condition.
• Collaborate with RN, CNAs, and other staff to ensure quality care.
• Maintain patient safety and infection control standards at all times.
