Job summary

Northbrooke Post-Acute in Jackson, TN is seeking a part-time Certified Nursing Assistant to support residents with daily activities and nursing care per the care plan. You will identify residents, record entries on flow sheets, and report changes to the nurse supervisor.

Transportation for activities and meal assistance are also part of the role. Responsibilities include monitoring resident safety, assisting with meals, hydration initiatives, and ensuring correct meal trays.

Qualifications

• High school diploma or GED.
• Must be a licensed Certified Nursing Assistant in Tennessee.
• Active BLS/CPR

Responsibilities

• Assist assigned residents with routine daily activities and nursing care per care plan.
• Identify residents before treatments or meals using wristband/photo.
• Record entries on flow sheets (ADLs, I&Os, meals, vitals, bowel/bladder) clearly.
• Report changes in resident condition to Nurse Supervisor/Charge Nurse promptly.
• Transports residents to activity locations as needed.
• Monitor and supervise residents to ensure safety at all times.
• Assist in passing meal trays and ensure correct trays to residents.
• Assist and participate in hydration program.
• Assist patients at meal times.
