Responsibilities

• Deliver daily therapeutic programming aligned with individualized care plans.

• Maintain activity calendars and adjust schedules based on census changes.
• Assist with transporting residents during in-facility and off-site events and support volunteers.
• Document participation and progress, support care coordination, ensure HIPAA and safety compliance.
