This role facilitates group and one-on-one activities tailored to residents’ interests, abilities, and psychosocial needs.

Responsabilidades

• Deliver daily therapeutic programming aligned to each resident’s care plan.
• Facilitate group and one-on-one activities tailored to residents’ interests and needs.
• Maintain activity calendars and adjust schedules as census changes.
• Assist with transporting residents during in-facility and off-site events.
• Support volunteers in contributing meaningful programming.
• Document participation and progress; contribute to care coordination.
• Ensure adherence to HIPAA, infection control standards, safety protocols, and QAPI requirements.
