Job summary

CommuniCare Rehab is seeking a Part Time Speech Language Pathologist (SLP) for Valley View Healthcare Center in Elkhart, IN. Our therapists are in-house team members dedicated to improving communication, swallowing, and cognitive skills for residents with complex conditions.

Primary duties include evaluating charts, developing treatment plans, conducting rehab services, and educating residents and families.

Qualifications

• Must have a Bachelor and/or a Master’s Degree in Speech-Language Pathology from an Accredited Program.
• Hold a valid or pending State Speech Language Pathology License.

Responsibilities

• Evaluate, review, and screen patients’ medical charts and physician-ordered evaluations; develop treatment plans.
• Plan, direct, or conduct rehabilitative treatment programs to improve communication, swallowing, and cognitive problems; administer and interpret speech tests.
• Instruct residents, families, and caregivers on monitoring speech, practicing new skills, diet recommendations, and safe swallowing techniques.
• Attend and lead interdisciplinary meetings to relay clinical findings and make recommendations to the rehab team; participate in family meetings/education.
