Job summary

Enhance Rehabilitation is currently seeking a qualified Speech Language Pathologist to join our dynamic team. In this role, you’ll evaluate and treat patients by following an individualized plan of care after an evaluation.

The SLP will provide patient care in a SNF setting, use standardized assessments, perform swallowing evaluations, and collaborate with families and the treatment team to monitor progress and adjust treatment plans as needed.

Qualifications

• Knowledge to provide patient care in SNF settings.
• Strong written and verbal communication with patients, families, and the treatment team.
• Experience administering standardized speech/language assessments, conducting swallowing evaluations, developing treatment plans, and monitoring progress.

Responsibilities

• Evaluate and treat patients by following an individualized plan of care developed from evaluation.
• Provide patient care appropriate in a SNF setting.
• Communicate with patients, families, and other treatment team members when gathering and sharing patient condition information.
