21st Century Home Health Services in Northern California is seeking a Speech Therapist/Language Pathologist (ST) for a fully in-person home health role in Solano County. You will provide skilled ST services in patients' homes, collaborating with the Case Manager and interdisciplinary team to evaluate and treat speech, language, cognitive, and swallowing disorders.

Requirements include a CA Speech Therapist License and BLS/CPR certification.

Qualifications

• Active CA Speech Therapist license and BLS/CPR certification required.
• Experience with in-home speech therapy and interdisciplinary teams preferred.
• Knowledge of culture and health care in home settings.

This opportunity is a fully in-person, home health role. Remote or hybrid work is not available. Candidates must be currently located in, or planning to relocate to, the following location:

Territory: Solano County: Vallejo, American Canyon, Hercules and minimal visits to Napa County*

What you’ll do as a Home Health Speech Therapist/Language Pathologies (ST):

In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication. You will serve as an educator, patient advocate and a member of the interdisciplinary care team

To be a success as Speech Therapist/Language Pathologist (ST)

• Current unrestricted CA Speech Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury; and of the influence of culture on health care.
• Knowledge of state and federal homecare regulatory guidelines preferred.
• Experience home health care is highly preferred
• Experience with high acuity patients is a plus
• Experience working with an interdisciplinary team is highly desired
• Case Management fundamentals is highly desired

• Prior to scheduling an in-person interview, all Candidates are required to complete a computer literacyand competencyassessment. $110,000 - $125,000 a year

Anticipated Wage range Speech Therapist: 110,000.00-125,000.00 Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits and 401k (Up to 4% match)
