Responsibilities

• Perform initial evaluations and establish treatment plans with guidelines.
• Review medical records and coordinate with attending physicians as needed.
• Set realistic, measurable short- and long-term goals for patients.
• Document progress and outcomes accurately in the medical record.
• Educate patients and families on rehabilitation concepts and home programs.
• Coordinate with interdisciplinary services for comprehensive care.
