Complete Care - Wisconsin is seeking a dedicated Part-Time Social Worker to identify, assess, and address the psychosocial, mental, and emotional needs of our residents while upholding their rights.

As the resident's primary advocate and emotional support liaison, you will conduct assessments, document Social History and psychosocial findings, contribute to MDS care planning, advocate for rights, coordinate discharge planning, and provide support to families.

Qualifikationen

• Bachelor's or Master's in Social Work required; Wisconsin license active.
• Minimum of 1 year supervised social work experience in health care; LTC/SNF preferred.
• Proficient in documentation and working within an interdisciplinary team; strong communication skills.

Aufgaben

• Assess psychosocial, mental, and emotional needs through visits and observation.
• Document Social History and Psychosocial assessments upon admission and thereafter.
• Integrate assessment findings into the MDS care planning process.
• Advocate for residents' rights, dignity, and self-determination.
• Coordinate discharge planning and post-discharge resources.
• Provide emotional support to residents and families.
• Other duties as assigned by the Administrator.
