Job summary

Newaygo County Mental Health is seeking a PART TIME Bachelor Level Social Worker/Case Manager to join the Adult Services Team. You will provide case management to adults with serious mental illness and/or developmental disabilities, including assessments, planning, advocacy, and coordination of transportation and medical consultations.

The role emphasizes person-centered planning, independent work, and strong documentation skills.

Qualifications

• Bachelor’s degree in Social Work or related field preferred.
• State licensure (LLBSW or LBSW) and a valid Michigan driver’s license.
• Strong belief in person-centered planning and experience delivering community-based services.
• Excellent verbal and written communication skills.
• Proficiency in computer skills and ability to manage case documentation.
• Ability to work independently, prioritize tasks, and handle interruptions.

Responsibilities

• Conduct behavioral assessments and develop individualized treatment plans.
• Coordinate services and monitor client progress; provide advocacy as needed.
• Arrange transportation and medical consultation when appropriate.
• Provide ongoing support and access to community resources.
• Ensure services align with person-centered planning and meet individual needs.
