Grace Community Care & Homes is developing its Adult Day Programs department and seeks a Social Worker Adult Day Programs. The role will contribute to person-centered services, reliable operations and accurate documentation.

Location is New Jersey with on-site or community assignments. Part-time; schedule discussed during hiring. Social-work credential appropriate to scope and geriatric experience required. Pay ranges from $27 to $35 per hour, contingent on qualifications and staffing readiness.

Qualifications

• NJ social-work credential appropriate to assigned scope.
• Adult or geriatric social-service experience.
• Required supervision.

Responsibilities

• Assess social needs and support participant and family care planning.
• Coordinate community resources and maintain social-service documentation.
• Collaborate with department leadership, intake, credentialing, billing and quality teams.
• Maintain confidentiality and communicate service gaps promptly.
