Job summary

Join Valley Manor Care Center in Montrose, CO as a Social Services Assistant. This role supports residents, coordinates transfers, and collaborates with the interdisciplinary team to maintain the highest practicable wellbeing.

It’s a hands-on opportunity to serve in a mission-driven healthcare setting with a strong community focus. Requirements include a high school diploma (Bachelor’s in Social Work preferred), state licensing where applicable, and about a year of long-term care experience.

Qualifications

• High school diploma or equivalent education required; Bachelor’s degree in Social Work or related field preferred.
• Licensed where applicable by the State; state requirements may dictate supervision and degree type.
• Minimum of one year experience in a long-term healthcare setting preferred.
• Knowledge of long-term health care and public health settings and familiarity with federal and state regulations for long-term care.

Responsibilities

• Supports Executive Director in care and treatment of residents to promote wellbeing.
• Serves as liaison to the community for the facility and represents the Social Service department.
• Coordinates resident transfers and discharges.
• Completes assessments for Care Planning, MDS, CAAS, PPS and other documents.
• Develops Care Plans and attends Interdisciplinary Care Conferences.

Pay Range: $23.65 - $26.25 per hour based on experience

Schedule: Monday - Friday 8-5

What You Bring (Requirements)

• High school diploma or equivalent education required; Bachelor’s degree in Social Work or related degree in social sciences preferred.
• Licensed where applicable by the State; State requirements may dictate supervision and degree type.
• Minimum of one year experience in a long-term healthcare setting preferred.
• Knowledge of long-term health care and public health settings and familiarity with Federal and State regulations and requirements for long-term care.

What You’ll Be Responsible For

The Social Services Assistant will support the Executive Director in the care and treatment of residents entrusted to the facility and to attain or maintain the highest practicable physical, mental and psycho-social well-being of each resident. To serve as a liaison and role model to the community for the facility. Represents the Social Service department, other care center staff, interdisciplinary team members and the facility as a whole to the community. Upholds the values and mission of Volunteers of America National Services to peers and the public. Customers include residents of the facility and their families, local healthcare professionals who work with the facility and staff, and the facility staff managers.

• Coordinates resident transfers and discharges.
• Completes assessments of residents as necessary for Care Planning, MDS, CAAS, PPS and other required documents. Identifies, provides and/or coordinates for the medically related social, psychological and spiritual needs of the residents.
• Develops Care Plans and attends Interdisciplinary Care Conferences for residents.
• Initiates reporting and documentation in the medical record.
• Develops and promotes the department through marketing efforts throughout the professional and social community.

Education

Required

High School or better.

Preferred

Bachelors or better in Social Work or related field.

Experience

Preferred

1 year: Experience in a long-term healthcare setting preferred.
