Total Care Therapy is seeking a part-time Speech-Language Pathologist to evaluate, diagnose, and treat communication, cognition, and swallowing disorders in older adults within assisted living and independent living communities in Northeast Ohio.

You will develop individualized plans, document in EMR, educate residents and staff, and collaborate with physicians and dietitians to ensure patient-centered care. Licensure required; CCC-SLP preferred; CPR/BLS; valid driver's license.

Qualifications

• Licensed Speech-Language Pathologist (SLP) in good standing.
• CCC-SLP credential preferred or eligible.
• CPR/BLS certification current.
• Valid driver's license with reliable transportation.

Responsabilités

• Evaluate residents and develop individualized speech therapy plans of care.
• Assess and treat speech, language, voice, cognitive-communication, and swallowing disorders.
• Perform dysphagia evaluations and implement swallowing interventions.
• Reassess progress and adjust treatment plans as needed.
• Complete timely documentation in the EMR.
• Educate residents, caregivers, and facility staff on communication strategies and home programs.
• Collaborate with physicians, nursing, dietitians, and interdisciplinary team for coordinated care.
