• Participates in school-based teams with staff and community agencies.
• Coordinates child welfare referrals and provides follow-up case management.
• Provides crisis intervention and follows up with referrals.
• Makes referrals to public or private agencies with appropriate follow-up.
• Liaises between school, family and community resources.
• Collaborates with families to strengthen home-school communication and reduce barriers.
