Qualifications

• Active SLP licensure in the state of assignment
• CCC-SLP certification preferred or may be required by state/contract
• CPR certification current (where applicable)
• TB questionnaire, PPD or chest x-ray if applicable
• Current health certificate per contract or state regulation
• Must meet all federal, state and local requirements
• Must be at least 18 years of age

Responsibilities

• Perform student evaluations in school setting
• Provide and document therapy services per plan of care
• Adhere to physician orders and clinical guidelines

Education

Active Speech-Language Pathologist Licensure in the state of assignment

CCC-SLP Certification (preferred)

CPR Certification (current)
