Clarinda Regional Health Center is seeking a Part-Time Social Worker to join our Social Work team. Work days are Monday-Friday; hours may vary. The facility operates 24/7, with shifts that may differ based on departmental needs.

The role includes psychosocial assessments, patient advocacy, discharge planning, and coordinating resources with the healthcare team to deliver compassionate care. Overtime eligibility and shift differentials may apply where appropriate.

Competenze

• Bachelor’s degree or associate degree with comparable work experience and a commitment to complete within negotiated timeframe preferred.
• Ability to work with physicians, mid-levels, and front-line staff in a collaborative manner to assist patients with exceptional care preferred.
• Minimum one (1) year experience in rural health preferred.
• Licensure through the Iowa Board of Social Work (preferred).

Mansioni

• Screen patients to identify psychosocial needs and intervention opportunities.
• Consistently interpret social, emotional, and family problems to attending physicians and other healthcare team members via rounds and documentation.
• Act as patient advocate for determining needed services and providing for assessed needs.
• Assist with discharge planning in collaboration with patients/families, healthcare team members, and community agencies in a concise and timely manner.
• Facilitates weekly Skilled Care Conferences and supports charity applications and financial assistance processes.
• Reviews and coordinates care plans and resources to fulfill departmental care initiatives.
