Qualifications

• Ability to work independently or as part of a team.

Responsibilities

• Complete and submit accurate MDS assessments.
• Oversee care plan process and coordinate conferences.
• Ensure ARD compliance with Medicare/Medicaid guidelines.
• Participate in monthly Quality Measure meetings.
• Collaborate with nursing leadership to improve documentation.
• Assist interdisciplinary team in Medicare coverage decisions.
