Qualifications

• Active New Jersey RN license or multistate (compact) RN license.
• Previous nursing experience.
• Experience in long-term care or sub-acute rehab setting preferred.

Responsibilities

• Monitor resident status and share updates with the care team.
• Administer medications and treatments with accurate documentation.
• Assist with admissions, transfers, and discharges to support smooth transitions.
• Provide nursing care following individualized care plans and procedures.
• Participate in assessments and help develop/update resident care plans.
• Communicate with residents, families, physicians, and team members about care needs.
• Respond to resident concerns to support comfort, safety, and well-being.
• Collaborate with nurses and other healthcare professionals to ensure quality care.
