Qualifications

Responsibilities

• Oversee coordination of nursing care, home health aide services, and therapies once approved.
• Maintain confidentiality of client and office records per HIPAA and Privacy Act PHI.
• Ensure effective communication among the care team for cohesive patient care.
• Perform additional duties as assigned to support the team and patient care.
