Job summary

Corewell Health is seeking a part-time RN case manager at Grant Medical. This role involves planning and prioritizing care for individuals and populations, coordinating with PCPs and other health professionals to optimize outcomes in a managed care environment.

Responsibilities include assessing educational needs, developing plans of care, and ensuring continuity of care across the continuum while managing resources and costs. BLS is required and CCM is preferred.

Qualifications

• Bachelor's degree or equivalent in nursing or a healthcare-related field.
• RN licensure in Michigan or state-specific requirement upon hire.
• 2 years of relevant clinical nursing and/or case management experience required.
• 5 years preferred; CCM certification preferred.

Responsibilities

• Plans and prioritizes care for individuals and populations.
• Develops plan of care and coordinates with PCPs and specialists.
• Assesses patient education needs and delivers appropriate teaching.
• Coordinates services to improve access and outcomes in a managed care setting.
