The role focuses on telehealth and virtual visits, coordinating with interdisciplinary teams to develop individualized care plans and ensure quality patient outcomes across home, hospital, and post-acute settings.

Qualifications

• Registered Nurse with current licensure and a Bachelor's degree or higher in Nursing.
• Preferred CCM or RN-BC certification (attainable within 36 months).
• Strong communication, interpersonal, and technology skills, including proficiency in Microsoft Office.

Responsibilities

• Develops individualized care plans by identifying barriers and setting specific goals.
• Assists in managing complex medical cases through thorough assessment and resource deployment.
• Facilitates safe transitions across care settings, including home, hospital, and post-acute facilities.
• Utilizes telehealth, virtual visits, and field-based support to deliver care.
• Collaborates effectively with interdisciplinary teams and engages patients and families to align care with their values and goals.
• Addresses gaps in care, leverages community resources, and ensures confidentiality and ethical standards.
• Documents activities accurately and works independently within scope of license.
