Job summary

Horizon House, Inc. is seeking a Residential Associate Clinician to join the Residential Service Group Home team at the Wilson Road location in Wilmington, Delaware.

The role supports participants transitioning to independence, conducts assessments, documents progress, and coordinates with families and community agencies. The position requires a Bachelor’s degree in a mental health field, knowledge of mental health systems, strong communication skills, and a valid driver’s license.

Qualifications

• Bachelor’s degree in counseling, psychology, social work, nursing, vocational/psychiatric rehabilitation, education or other mental health field from an accredited college or university.
• Knowledge of mental health and substance abuse systems and supports.
• Ability to work independently.
• Good communication skills both oral and written.
• Possess a valid driver’s license with an acceptable driving record.
• Direct experience in mental health services with emphasis on direct care in a community-based setting.

Responsibilities

• Complete the initial and annual participant assessments and develop the participant’s recovery plan from a person-centered perspective.
• Assist with the presentation in case conference for assigned participants.
• Responsible for the care and maintenance of case records for all assigned participants.
• Act as a liaison and consult with community agencies and families to maintain coordination in the recovery process.
• Provide ongoing assessment of participant’s mental illness symptoms and their responses to recovery to ensure that immediate and appropriate interventions are provided.
• Participate in staff planning, training and internal/external administrative meetings.
