Job Overview:
We are seeking a dedicated and knowledgeable Nutritionist to join our care team. The ideal candidate will bring a strong foundation in dietetics and physiology, with the ability to develop personalized care plans, conduct comprehensive assessments, and collaborate within an interdisciplinary healthcare team. Experience with weight management, chronic disease support, and long-term care environments is a plus.
This role combines patient-facing telehealth counseling with opportunities to support care coordination and quality improvement efforts within our virtual nutrition program.
Key Responsibilities:
Conduct comprehensive nutrition assessments to evaluate patient health status and dietary needs
Provide individualized nutrition counseling sessions via telehealth
Develop and implement customized care plans that address chronic conditions, weight loss goals, and specific dietary requirements
Identify nutritional deficiencies through diagnostic evaluations and recommend appropriate interventions
Offer practical education and guidance on healthy eating, food budgeting, meal planning, and lifestyle behavior change
Provide education on specialized nutrition support such as tube feeding when needed
Collaborate with Greens Health’s care coordinators, case managers, and clinicians to deliver integrated, whole-person care
Supervise dietary staff (if applicable) to ensure adherence to nutrition protocols and quality standards
Document patient progress accurately in our electronic health record (EHR) system
Stay up to date on current research and trends in nutrition, dietetics, and chronic disease management
Requirements:
Bachelor’s or Master’s degree in Nutrition, Dietetics, or a related field
Licensed Dietitian (LD/RD) or Certified Nutrition Specialist (CNS)
Strong knowledge of physiology and its application in dietary planning
1–2 years of experience in clinical nutrition counseling (telehealth experience preferred)
Familiarity with chronic disease prevention, weight loss programs, and long-term care nutrition strategies
Strong communication skills — both verbal and written — with a patient-centered, culturally responsive approach
Experience in underserved community settings or FQHCs is a plus
Comfortable with virtual care delivery and digital documentation platforms
Supervisory skills or prior experience leading dietary staff is a bonus, but not required
