Trinity Health is seeking a Home Care Coordinator for a part-time remote role. You will serve as the key liaison between hospital discharge planning and post-acute home care services, ensuring smooth transitions for patients and families.

This position emphasizes collaboration with hospital staff, education of patients, and building strong referral relationships. The hybrid schedule includes Sundays 8:30 AM - 5:00 PM ET with training on Saturdays for about 8 weeks after hire.

Qualifications

• Bachelor's degree or equivalent experience is required.
• Minimum of 2 years of experience in acute care, care coordination, customer service, sales/marketing, or related role.
• Excellent verbal and written communication skills.
• Preferred home care/hospice/medical background is desirable.

Responsibilities

• Serve as primary contact for hospital case managers and care teams.
• Coordinate referrals from acute care to home care or hospice.
• Educate staff, patients, and families on services and options.
• Document assessments and care plans per policy.
• Obtain physician orders and coordinate initiation of services.
• Transmit clinical documentation to agency intake.
• Explain insurance coverage details and financial responsibilities.
• Maintain ongoing communication with physicians, intake teams, families, and care partners.
• Participate in quality improvement initiatives and support new program development.
• Build referral relationships through hospital rounding and provider visits.
• Uphold confidentiality, compliance standards, and organizational values.
