Nutritious Lifestyles, Inc. is seeking a part-time Registered Dietitian Nutritionist (RDN/LDN) to deliver individualized nutrition care to residents.

You will conduct assessments, provide 1:1 consultations, and develop care plans tailored to diverse conditions such as diabetes, dialysis, cardiac care, and wound healing. Qualifications

• RDN/LDN credential required.

Responsibilities

• Conduct nutrition assessments, consultations, and follow-up evaluations.
• Provide individualized nutrition care plans and 1:1 resident consultations.
• Address diverse clinical conditions including dialysis, diabetes, cardiac care, wound healing, and tube feeding.
• Develop and implement individualized nutrition care plans.
• Monitor nutritional status and recommend interventions.
• Collaborate with healthcare professionals to improve nutrition outcomes.
• Apply the Nutrition Care Process to deliver high-quality care.
• Use Food as Medicine to improve residents' quality of life.
