Artesia Palms Care Center is seeking a dedicated Dietitian to join our long-term care team. The role focuses on assessing nutritional needs, developing tailored care plans, and educating residents on healthy eating.

You will collaborate with healthcare professionals, monitor progress, and stay current with clinical nutrition practices. Pay rate is $55 per hour, reflecting the essential nature of nutrition care in our center.

Qualificações

• Strong background in dietetics and physiology, with ability to assess nutritional needs and develop tailored nutrition care plans.

Responsabilidades

• Conduct comprehensive patient assessments to evaluate nutritional needs based on medical history and current health status.
• Develop, implement, and monitor individualized care plans that include dietary recommendations and interventions.
• Monitor patients' progress and adjust meal plans as needed.
• Educate patients on proper nutrition, including food choices, portion sizes, and meal timing.
• Collaborate with healthcare professionals to ensure comprehensive patient care.
• Utilize physiology knowledge for effective dietary recommendations.
• Perform diagnostic evaluations to assess patients' nutritional status.
• Offer health coaching to support patients in achieving their dietary goals.
• Develop preliminary and comprehensive assessments of the dietary needs of each resident.
• Provide nutrition expertise and serve as a resource to other members of the health care team.
• Participate in the planning, conducting, and scheduling of timely in-service training classes that ensure a well-educated dietary service department.
• Maintain current knowledge in clinical practices of nutritional therapies and treatment protocols.
• Ensure that all dietary personnel are aware of the Care Plan and that Care Plans are used in planning daily dietary services for the residents.
• Review nurses’ notes and meets with Direct Care Staff to determine if the Care Plan is being followed.
• Discuss problem areas with the Director of Nursing.
• Review and revise Care Plans and assessments as necessary, at least quarterly and annually.
