Naamans Creek Country Manor in Garnet Valley, PA is seeking a Part-Time Registered Dietitian to join our team. This role offers approximately 16–24 hours per week, Monday through Friday, with scheduling flexibility based on the facility's needs.

The RD will assess nutritional needs, develop care plans, and collaborate with nursing and dietary staff in a busy long-term care setting. Candidates should be licensed in Pennsylvania, with CDR registration and RD status; experience in long-term care

Formación

• CDR registration required.
• Active Pennsylvania Dietitian Licensure required.
• Must be a Registered Dietitian (RD).
• 1–2 years of long-term care experience preferred.
• Experience in CHF-certified facilities preferred.
• Strong communication, organization, and time-management skills.
• Ability to work independently and manage multiple priorities.

Responsabilidades

• Attend AM and clinical meetings on site.
• Complete nutritional assessments for new admissions and readmissions.
• Complete MDS assessments according to schedule.
• Review weights weekly, monthly, and daily and assess significant changes.
• Develop and maintain individualized nutritional care plans.
• Conduct monthly assessments for Nutrition at Risk residents, including those with wounds.
• Monitor therapeutic and modified diets for accuracy.
• Communicate nutritional recommendations with nursing and dietary staff.
• Maintain accurate and timely clinical documentation.
• Collaborate with the interdisciplinary team to support quality resident care.
