Job summary

HealthPro Heritage is seeking a part-time Licensed Physical Therapist Assistant to provide outpatient therapy services for adult residents in an assisted living facility in Fairfield, IL. You will support PTs with treatment plans and deliver direct care in an onsite clinic setting.

Responsibilities include documenting progress, educating patients and families, and collaborating with a multidisciplinary team to achieve optimal outcomes.

Qualifications

• Associate’s degree in Physical Therapist Assisting from an accredited program.
• Valid state licensure as a Physical Therapist Assistant, or license eligible
• Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills.

Responsibilities

• Patient Care: Assist in implementing treatment plans designed by Physical Therapists and provide direct patient care including therapeutic exercises, manual therapy, and modalities.
• Treatment and Education: Monitor and document patient progress, report observations to the Physical Therapist, and educate patients and families on treatment protocols, exercises, and self-care techniques.
• Documentation: Accurately document patient treatments, progress, and responses in compliance with facility policies and regulatory standards.
• Collaboration: Work closely with Physical Therapists, other healthcare professionals, and support staff to ensure coordinated patient care and effective treatment outcomes.
