Job summary

Concept Rehab, Inc. is seeking a Part Time Physical Therapist Assistant (PTA) for 20-25 hours per week, located in Centerville, Ohio. The PTA will treat patients under the supervision of a Licensed Physical Therapist, engage in communication with families and facility staff, and maintain documentation of services provided. Required qualifications include an Associate Degree in Physical Therapist Assistant and licensure as a Certified Physical Therapist Assistant. Qualifications

• Licensed in the state(s) where services are being provided as a Certified Physical Therapist Assistant.

Responsibilities

• Treat patients according to a treatment plan under supervision.
• Communicate with the Physical Therapist and families regarding patient progress.
• Participate in interdisciplinary team meetings.
• Record treatment and progress notes.
• Instruct patients’ families in maintenance programs.
• Comply with the Licensure and Certification Standards.
