Job summary

Wellstar Spalding Out-Patient Rehab Center is seeking a part-time Physical Therapist to assess, plan, and provide rehabilitative care for patients, with a focus on mobility, pain relief, and functional outcomes. The role requires a Doctorate/Masters/Bachelors in Physical Therapy and current PT licensure along with BLS.

Responsibilities include developing treatment plans, documenting progress, educating patients, and coordinating with physicians and staff.

• Basic computer skills for documentation and scheduling.

Responsibilities

• Implements and monitors the physical therapy treatment plan to meet patient goals.
• Collaborates with physicians to evaluate and adjust plan of care.
• Documents evaluations, plans, and sessions per department guidelines.
• Provides patient and family education and discharge instructions.
• Acts as a liaison to refer patients to community resources as appropriate.
• Participates in staff meetings and mandatory training.
