Qualifications
- Graduated from an accredited Physical Therapy (PT) college or university.
- Valid Physical Therapy (PT) license for state practicing.
- Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
- CPR Certification.
Responsibilities
- Provide direct patient care and PT assessments for referred cases.
- Document patient progress in PCC EMR.
- Collaborate with the care team to optimize rehabilitation plans.
