Job summary

Qualifications

• Must be licensed or eligible in Texas.

Responsibilities

• Conducts and documents psycho-social assessments to interpret patient needs.
• Develops and evaluates treatment plans with the care team.
• Provides care plans including crisis intervention, education, advocacy, and counseling.
• Liaisons with community resources and staff for smooth transitions and placements.
• Collaborates with healthcare team to improve patient outcomes.
• Participates in performance improvement initiatives.
