Job summary

New Haven Center for Nursing & Rehab in New Haven, CT seeks a Part-time or PRN Physical Therapist. New graduates are welcome to apply. Located at 181 Clifton St, New Haven, CT 06513, this 150-bed SNF offers a supportive therapy team and a progressive, patient-focused environment.

Responsibilities include evaluating and planning treatment, delivering care per the plan, educating patients and families, monitoring progress, and completing required documentation in line with regulations.

Qualifications

• Evaluate and treat residents per established plan of care.
• Complete documentation in accordance with policy and regulations.
• Instruct patients, families and caregivers on treatment plans.

Responsibilities

• Evaluate, plan and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing
• Assure all treatment is delivered in accordance with an established plan of care
• Provide clinical support and instruct patients, families, and caregivers
• Monitor patient response to treatment intervention
• Complete required forms and documentation in accordance with company policy and state/federal regulations
