Job summary

Aveanna Healthcare is seeking an Occupational Therapist (OT) to join our team as a Part Time PRN therapist in Johnson and Linn Counties, IA. You will assess, teach, and deliver therapy interventions directed by the Physician as established in the home health plan of care, collaborating with interdisciplinary team members for optimal patient outcomes.

The role requires an OT degree, current unrestricted state license, and CPR.

Qualifications

• Degree in Occupational Therapy from an accredited university/program.
• Current, unrestricted state license as an Occupational Therapist in the state of practice.
• Valid CPR.

Responsibilities

• Perform initial and ongoing assessments to evaluate mobility, coordination, strength and current level of function.
• Deliver occupational therapy services to comply with prescribed physician orders.
• Provide a written evaluation and plan of care defining occupational therapy needs, goals, progress and outcomes.
• Maintain plan of care by timely documentation and discharge planning as appropriate.
• Discharge planning and home exercise plan education for patient and caretakers.
• Coordinate services with health care team and family; supervise OTA/HHA as applicable.
• Complete OASIS assessments where allowed by state practice.
