Job summary

CareMed Group in Dayton, OH seeks a Nurse Practitioner to provide primary and preventive care for residents in skilled nursing facilities, managing acute changes and coordinating with physicians.

Qualifications

Responsibilities

• Provide primary and preventative care for long-term care residents in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Advanced care planning and chronic care management.
• Prescribe medications, including controlled substances, per applicable law.
• Order, review, and follow up on treatments and diagnostics.
• Communicate with nursing home staff about patient changes.
• Collaborate with primary care physicians.
