Job summary

CareMed Health Solutions is seeking a Nurse Practitioner to deliver medical services to residents in long-term care facilities. You will provide primary and preventive care, manage acute changes, and collaborate with an interdisciplinary team to optimize patient outcomes.

Qualified candidates will hold an unrestricted NP license in Kentucky, board certification, DEA licensure, and experience in geriatric or AC settings.

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board Certification in family, adult, or geriatric specialty.
• DEA license and active credentials.
• Clinical background in adult, family or geriatric care.
• Experience in long-term care setting.

Responsibilities

• Provide primary and preventative care for long-term care residents in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Advanced care planning and chronic care management.
• Prescribe medications, including controlled substances in accordance with KY Law.
• Order, review and follow-up with treatments and diagnostic testing.
• Communicate with nursing home staff on a regular basis to discuss condition changes and care plans.
• Collaborate with primary care physician.

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with KY Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of Kentucky.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
• Psych experience a plus
