Job summary

CareMed Health Solutions is seeking Nurse Practitioners to deliver primary and preventive care to residents in long-term care facilities. You will manage acute changes, perform advanced care planning and coordinate with an interdisciplinary team to ensure residents receive optimal care.

Applicants must hold a KY NP license with board certification and DEA licensure. Experience in geriatric/long-term care settings is preferred; a strong EMR background is required.

Qualifications

• Current, unrestricted NP license in KY.
• Board Certification required.
• DEA licensure required.
• Experience in adult/geriatric care and long-term care settings preferred.

Responsibilities

• Provide primary and preventative care for long-term care patients in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Advanced care planning and chronic care management.
• Prescribe medications, including controlled substances per KY law.
• Order, review and follow up on treatments and diagnostics.
• Communicate with nursing home staff regarding changes in condition.
• Coordinate with the care plan with interdisciplinary team.
• Collaborate with primary care physician.

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with KY Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of KY.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
• Basic coding knowledge preferred, not required.
