NYU Langone Hospital - Long Island in Patchogue is seeking an Advanced Practice Provider Nurse Practitioner for primary care on a part-time basis. You will assess patients, plan and implement care, and collaborate with a multidisciplinary team to promote wellness.

This role requires a Master's degree in NP, NYS NP and RN licenses, BLS, and strong communication. The successful candidate will provide patient- and family-centered care in a busy outpatient setting.

Qualifikationen

• Master's degree from an accredited Nurse Practitioner program.
• Current NYS RN license.
• Current NYS NP license.
• Current Nurse Practitioner certification.
• Excellent communication skills (oral & written).
• Basic computer skills.
• BLS certification.
• For Ostomy, WOCN certification within 2 years of hire.

Aufgaben

• Performs duties as assigned.
• Plans and organizes care to meet patient needs per protocols.
• Collaborates with interdisciplinary team to implement the therapeutic plan of care.
• Participates in development of standards and patient education materials.
• Maintains timely communication with family, staff and other disciplines.
• Prescribes in-patient and discharge medications per NYS law.
• Ensures continuity of care across services and settings.
