Qualifikationen

• Master's degree graduate of an accredited Family or Pediatric Nurse Practitioner program is required.
• Current NYS Registered Nurse License required and Current NYS Nurse Practitioner License required.
• Current Nurse Practitioner Certification required. Excellent communication skills (oral & written), interpersonal and organization skills required.
• Basic Life Support Certification through the American Heart Association.

• Performs other duties as assigned.
• Plans and organizes care to meet individual patient needs and ensure appropriate clinical resource utilization per protocols.
• Works with the interdisciplinary team to develop and implement the therapeutic plan of care for groups of patients and ensure communication with families.
• Participates in development and revision of nursing standards and patient education materials.
• Communicates with families, staff, and other disciplines to maintain timely and appropriate information exchange.
• Ensures continuity of care across the continuum of services, including home care and follow-up appointments.
• Evaluates and revises treatment plans according to clinical standards and practice protocols.
• Considers patient age and cultural needs in all care provided.
• Promotes professional growth and maintains current expertise in area of practice.
