Infinite Medical P.C. is hiring a Primary Care Nurse Practitioner to provide on-site care in long-term care facilities in Omaha, Nebraska. This part-time role is 2 days per week and offers a salary between $60,000 and $90,000 per year.

Candidates must hold an active and unrestricted NP/PA or physician license with Nebraska licensure in good standing and at least six months in long-term care. The position emphasizes patient relationships, collaborative care, and high-quality elder care.

Qualifications

• Active and unrestricted NP/PA or physician license.
• State license in good standing in Nebraska.
• Minimum 6 months of experience in long-term care.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and goals.
• Ensure a safe, respectful, and supportive environment.
