Job summary

Hattersley Group Practice is seeking a Part-Time Practice Nurse to support clinical care across our patient population. You will assess patients, order and interpret investigations, perform procedures, and contribute to care planning, discharge, and referrals in line with NICE guidelines.

You will work with a multidisciplinary team, provide patient education on medications and self-care, and ensure effective communication with patients and carers.

Responsibilities

• Assess, order and interpret investigations, undertake practical procedures, diagnose, plan and implement treatment.
• Provide information on prescribed or over-the-counter medications and interactions.
• Deliver care in accordance with NICE guidelines and ensure safe discharge or referral.
• Communicate effectively with patients and carers, considering language and cultural needs.
