Job summary

Adfinitas Health is seeking a qualified NP/PA for a part-time Post-Acute role in the Western Maryland region, based in Silver Spring, MD. The position focuses on providing high-quality post-acute medical services across skilled nursing, rehabilitation, long-term care, and assisted living facilities.

Candidates should have 1+ year in an acute APP role, be PA-C or NP graduates, and hold current licensure with the ability to obtain facility credentialing.

Qualifications

• 1+ years of experience as an Advanced Practice Provider in an acute setting, preferably long-term care or skilled nursing facility.
• Licensure to practice medicine in good standing in the state
• Ability to be credentialed at designated locations
• Demonstrated clinical competency in evaluation, diagnosis and treatment

Responsibilities

• Perform exams and develop treatment plans.
• Oversee care episodes and review notes per facility policy.
• Facilitate patient transitions between care settings (admitting, transferring, discharging).
• Collaborate with a multidisciplinary team.
• Submit accurate charges for services performed
