Qualifications

• 1+ years of experience as an Advanced Practice Provider in an acute setting (preferably long-term care or skilled nursing).
• Graduate of an accredited PA-C or NP program.
• Licensed in good standing; applicable state licensure.
• Active DEA license and CDS certification in good standing.
• Capable of credentialing at designated locations.
• Strong clinical skills with evaluation, diagnosis and treatment abilities.
• Ability to work autonomously and manage time effectively.
• Excellent organizational and communication abilities.
• Comfortable in a fast-paced, high-stress environment.
• Proficient with EMR systems and documentation.

Responsibilities

• Perform exams, assessments and develop treatment plans.
• Oversee care episodes and review notes for facility patients.
• Facilitate transitions between care settings (admission, transfer, discharge).
• Collaborate with a multidisciplinary team to coordinate care.
• Submit accurate charges for all services rendered.
