Qualifications

• Graduate of an accredited program in NP or PA (PA-C or NP).
• 1+ years of experience as an Advanced Practice Provider in an acute setting.
• Licensure to practice medicine in good standing in the state.

Responsibilities

• Perform exams, assessments, and develop treatment plans.
• Oversee care frequency and review notes for episodes of care in SNF settings.
• Facilitate transitions between care settings (admission, transfer, discharge).
• Collaborate with multidisciplinary team members.
• Submit accurate charges for professional services.
