Encore Village of Schaumburg is seeking a Part Time LPN to perform day-to-day nursing activities in our skilled nursing department. The LPN will provide direct resident care within the licensed scope, following federal, state, local guidelines and our facility policies.

Responsibilities include resident assessments, monitoring for changes, administering medications, and triaging emergencies. Candidates must hold an IL LPN license, graduate from an accredited LPN program, have at least 2 years in

Formación

• Current LPN license in Illinois.
• Graduation from an accredited LPN program.
• Minimum of 2 years experience in long-term care or skilled nursing.
• Current CPR Card.

Responsabilidades

• Performs resident assessment through direct physical assessment, monitoring, reviewing records, and information from the interdisciplinary team.
• Monitors residents for changes in condition and notifies physicians/families; transcribes orders.
• Administers medications according to facility policy and monitors for adverse effects.
• Triages emergencies and provides appropriate first aid; completes reports and consults the ADON or charge nurse.
