New Horizons Community Support Services, Inc. seeks a Board-Certified Physician (MD or DO) to serve as a Specialized Healthcare Consultant for our Healthcare Home Program.

This part-time role (about 20 hours per month) complements an existing practice and involves care coordination without direct patient panels or prescribing. The SHC collaborates with an interdisciplinary team to improve physical health outcomes for adults with behavioral health conditions, with no after-hours or on-call

Qualifikationen

• MD or DO degree from an accredited medical school.
• Board Certified in Family Medicine, Internal Medicine, or related primary care preferred.
• Current Missouri medical license in good standing.

Aufgaben

• Provide clinical consultation to align Healthcare Home services with medical standards.
• Collaborate with Nurse Care Managers and a Care Coordinator for enrollees’ physical health needs.
• Consult with psychiatrists and the treatment team on health and wellness concerns.
• Support care coordination with primary care practices, specialists, and hospitals.
